PLEASE NOTE THAT WE CANNOT PROCESS YOUR MECHANIC’S LIEN REQUEST UNTIL WE RECEIVE THE FOLLOWING ITEMS:
- Copy of Preliminary Notice (also any Amends and/or Adds to the original Preliminary Notice)
- Copy of Preliminary Certificate of Mailing or Certified Mailing and/or Return Receipt (proof of mailings)
- Certificate of Mailing Affidavit (from the company who processed and served your Preliminary Notice)
- Copy of Contract or Scope of Work (if no Contract)
- Copy of all invoices that calculate to the total Lien amount
- Copy of change orders that calculate to Lien amount (if applicable)
